HPV Vaccination Clinic
When
- Wednesday, May 7, 2008 from 9:00am - Noon and 1:00pm - 4:00pm
- Please Note: Once you have received the vaccination you will be required to stay at the clinic for 20 minutes for observation.

- Women's Clinic: Second floor, Student Health Services building
Cost
- $155 (fee can be placed on the Student Fee Bill or paid at the time of service)
What to bring
- Your UCONN Student ID Card
- Documentation of any previous Gardasil vaccination that was done outside of Student Health Services (we will not administer your 2nd or 3rd vaccination without it).
- Vaccination Permission Form (if you will be under age 18 at the time of immunization)
Your Health Insurance Card
If you are covered by Anthem Blue Cross/Blue Shield, HealthNet, Chickering or TriCare we will bill claims directly to your insurance. These are the only carriers that we will bill directly. Your insurance card must be presented at the clinic. For other insurance plans, please check with your insurance company regarding reimbursement and covered service.

Important Information
- To reserve your dose of vaccine, fill out the on-line reservation form.
- If you need to cancel a reservation you already made, please call 486-4837 before April 29th. If you reserve a dose and fail to appear or
cancel, you will be billed the full vaccination charge ($155).
